Please click here to download the Registration Form.  The registration for the Gala Night is closed as of July 18, 2008, because we are now completely full.  However, please continue to register for the Reunion Night as well as the Continuing Education meetings.

Make check or money order payable to VPFNC (please write IC-VPDP 2008 on the check or money order memo line) and send with completed Registration Form to:

*Registration Form will not be processed unless accompanied by a check or money order.  If already paid online, please indicate so on the Registration Form.

*If both husband and wife are health professionals, please each register in a separate form and send to his/her respective Association.

Receipt
A receipt will be sent by mail (or email if available) to all those who have registered and fully paid their fees.

Refunds
If the request for refund is received after May 31, 2008, only 50% of the Registration Fee will be refunded.  After July 15, no refund will be made.
